How do I make an ESA claim?
You can file a claim online or get a copy of the claim form at ontario.ca/ESAclaims.
Or you can get the form at a ServiceOntario Centre.
If you have a paper copy of the form, you can fill it out and file it by:
- taking it to a ServiceOntario Centre
- faxing it to the Ministry of Labour at 1-888-252-4684
- mailing it to the Provincial Claims Centre, 70 Foster Drive, Suite 410, Sault Ste. Marie, Ontario P6A 6V4.
